Gabriel T. Swieszcz is a dedicated Population Health Specialist with over 3 years of experience in care management, community outreach, and provider contracting. Currently, he is interning with the Center For Food Equity In Medicine, where he contributes to advancing healthcare initiatives focused on food equity. Gabriel is set to graduate in December with a Master’s Degree in Public Health and Administration from Benedictine College.

His career spans roles at organizations such as HarmonyCares Medical Group, Engaging Solutions, and VNA Healthcare, where he has excelled in coordinating patient care, managing strategic contracts, and improving patient satisfaction. He is proficient in electronic health records (EHR) and customer relationship management systems like Salesforce.
